Golf Course Report Card Cancel The title field is required!

   I have been asked to put together a report card per se, as it relates to golf maintenance.
   Kind of an evaluation tool to be filled out monthly or maybe quarterly.  Something for me to give myself a grade on the overall conditions of the golf course, to note any successes and/or failures.  A self review to be discussed with General Manager.  Does anyone have anything like this they use? Would you mind sharing? I'm just looking for some examples to determine how best to proceed with this.

   Anthony this sounds like the PDA program GCSAA pushed through many years ago. It failed miserably but if you can find the outline of it , it should give you some kind of guideline to work with.
